    Applicants who have attended an Ontario university must use the online transcript request. Transcripts sent directly from the registrar’s office will not be used. Certain exceptions apply. Please read the instruction booklet for full details.

I have applied by paper but I cannot find my Acknowledgement/Amendment Form. Can you give me my PIN/password?

We cannot provide your PIN/password by telephone or by email. We can re-issue the Acknowledgement/Amendment Form to the mailing address in your file. If the mailing address on file is different than your current mailing address, you will need to fax a signed and dated request with your name, OUAC Reference Number, new address and the request to re-issue the Acknowledgement/Amendment Form to the new address.
