Supplementary forms can be downloaded from the TEAS website at www.ouac.on.ca/teas/. You will need Adobe Reader in order to download and print the PDFs. Include your IT or Reference Number, and send all completed forms directly to the appropriate Faculties of Education by the appropriate deadlines.
