  • Ontario University Transcripts

    Applicants who have attended an Ontario university must use the online transcript request. Transcripts sent directly from the registrar’s office will not be used. Certain exceptions apply. Please read the instruction booklet for full details.

Common Questions

How can I be sure that my TEAS application was submitted successfully?

The actual “Submit” process consists of the following two steps:

  1. You will review all information you entered in the application, and accept a declaration that the application is complete and accurate by clicking on “I Verify and Agree”.
  2. You will enter your payment information and click on “Finish” to submit your completed application to TEAS. If you have successfully submitted your payment, you will receive a six-digit TEAS reference number. Please keep a record of this number, as you will need it for future communication with TEAS.
