Application Components
The following is a brief summary of the information that is required to complete each section. Review instructions thoroughly as you are responsible for submitting correct and complete information. Instructions can be found in the help windows provided in most sections of the online application.
| Title headers | “Introduction” (includes information about the application process), “Review and Submit” (allows you to review the information that you have entered, notes missing information and then is used to submit the completed application) and “Log Out” (use any time you wish to log out of your application). |
| Personal information |
This section collects your identifying information, which is fundamental to the application process. You must provide your full legal name, date of birth, citizenship, and language information. |
| Address information |
Home, mailing and email details are gathered in this section. |
| Choices | Select the rehabilitation science programs to which you are applying. |
| Personal Submissions |
Some of the universities/programs require you to answer additional questions or to submit an essay. All of these requirements are found within ORPAS. It is important to note that there are limits to the length of your answers. |
| Referees | You may be required to ask individuals (a.k.a. “referees”) to provide a reference on your behalf using the Confidential Assessment Form. You must provide their names and addresses. |
| Postsecondary Education |
You will be required to enter information about the universities and/or colleges that you have attended or are currently attending. |
| Transcripts | If you have attended universities in Ontario, you must request your transcripts by completing this section. The OUAC has been processing transcripts electronically since 1997 with the Ontario universities. |

| About ORPAS | This section provides a brief description of the ORPAS service and its governance. |
| Confidentiality | This section provides information about the security of the OUAC website and how the OUAC uses the data that it collects. |
