You may only enter 1 person on the application in the R4 (Confidential Clinical Referee) position. For any additional clinical references, please email firstname.lastname@example.org. You will be required to give us the name and contact information of the additional clinical referee.

If my marks from an exchange program are entered on my “home” university’s transcript, do I need to send a transcript from my “exchange” university?
Yes. Transcripts are required from all institutions attended.
My credit card statement shows “OUAC Guelph”. I did not apply to the University of Guelph. Is there a problem?
“OUAC Guelph” refers to the Ontario Universities’ Application Centre. We are located in Guelph, Ontario. The OUAC processes all applications and payments. The specific universities you applied to will not be listed on your credit card invoice.
Error messages are problems within the online application that must be corrected. You will not be able to submit your application until these are corrected.
